How to Use the Feedback Forms
This page explains the difference between the two feedback forms and helps you choose the right one. Please read through before submitting your feedback, so your message reaches me in the way you intend.
Anonymous Feedback Form
The anonymous form is designed for open, honest feedback when you’d prefer not to attach your name. It’s a great option for sharing thoughts that might feel awkward, personal, or difficult to say directly.
When to Use the Anonymous Form
When you want to give frank or sensitive feedback
When your message is more about helping me improve than starting a conversation
When staying unnamed helps you be more open and clear
What NOT to Include
To keep things respectful and safe for everyone, please avoid:
Personal information about yourself or others
Confidential or restricted workplace details
Anything that could identify you if you don’t want it to
Offensive, harmful, or inappropriate content
Privacy Notice
This form does not collect your name or contact details.

Direct Feedback Form
The direct form is for situations where you’re comfortable sharing who you are and you’re open to further conversation if needed. It allows me to understand the context better and follow up for clarity when appropriate.
When to Use the Direct Form
When you want a response or follow-up
When your feedback is more detailed or needs context
When your message might start a two-way discussion
When you’d like to be acknowledged personally
What Information You’ll Provide
The direct form includes these fields:
Name – so I know who the feedback is from
Email Address – for follow-ups and/or confirmation
Message – your feedback, thoughts, or suggestions
Follow-Up & Confirmation
After submitting the direct form:
A confirmation email will be sent to let you know your message was received
I may contact you if I need clarification or would like to continue the discussion
(You won’t be contacted unnecessarily.)
Choosing the Right Form
If you want to stay completely anonymous → Use the Anonymous Form
If you want open communication or follow-up → Use the Direct Form
Both options are equally valued — choose the one that feels right for you.
